New Student Registration
You may register a new student with Iveth Vazquez in the Mount Ogden Junior High Counseling Office. Please call 801 737-8575 to schedule a registration appointment when possible to ensure availability.
You will need to provide the following documents at the time of registration:
- Birth Certificate
- Shots Record
- Proof of Address (a bill/piece of mail with your name and address on it)
- Previous School Records
The following items are required by the Ogden School District before registering for school:
Parent: You must have legal guardianship of your student to register them in school. If not, please process your child through “Student Services” at the District Office first.
